1. WHAT ARE DERMAL FILLERS?

Dermal fillers are injectable introduce into the skin to fill up and lift lines, wrinkles and skin folds and to replace volume loss. They are also used to augment or enhance the shape and volume of the forehead, the nose, the cheeks, the chin and the lips. Besides filling up lines and creating volume, dermal fillers stimulate collagen growth and give hydration to the skin.

2. WHAT ARE THE DIFFERENT FILLERS?

Dermal fillers are grouped into 3 main types: Temporary fillers, semi-permanent fillers and permanent fillers. In our dermatology clinic, Dr Joyce Lim uses both temporary and semi-permanent fillers.

Temporary fillers are those that when injected into the skin will be slowly removed by the skin and they last up to a year from initial injection. They do not accumulate in the skin. Temporary fillers are made of collagen and hyaluronic acid (the HA fillers), both are which are harmless to the skin.

Semi-permanent fillers are often known as stimulatory fillers and they stimulate collagen growth and create volume in addition to filling up wrinkles and folds. They last for 1-2 years from initial injection. The common semi-permanent fillers are Radiesse and Sculptra, both of which are FDA and HSA approved in Singapore.

Permanent fillers stay in the skin for up to 5 years and they have been associated with nodules formation.

3. WHAT ARE DERMAL FILLERS USED FOR?

Dermal Fillers are most commonly used to rejuvenate and restore volume in an ageing face or a thin face from weight loss. They are also injected to fill up lines and folds eg the nasolabial folds (the folds from the nose to the corner of the mouth), the marionette lines (the lines that runs from the corner of the mouth downwards giving a sad look), the tear trough (the hollows under the eyes) and deep forehead or neck lines.

Dermal fillers enhance and define the face, the cheeks, the nose, the lips, the chin and the forehead. They are injected into sunken cheeks for fuller facial contours, fill out the pre-jowl areas to improve irregular jaw lines caused by gravity, smooth out hollows under the eyes or tear troughs for fresher looking eyes, and making the nose more attractive by straightening out noses that are deviated or have a bump or to increase the height of the nose.

Dermal fillers creates a more youthful look by creating a more oval face and improves nose, lips, cheek and chin volume and shape.

Dermal filler like sculptra acts as scaffolding and allows collagen to build up, replenishes the lost volume.

4. WHAT CAN I EXPECT FROM DERMAL FILLER INJECTION?

Treatment takes approximately 30 minutes. With hyaluronic acid( HA) fillers, the area may swell over the next day and this will subside the following day.

Result lasts from one to two years, depending on the area being treated, the amount and type of dermal filler used, the depth of the lines or volume loss, the condition of the skin and the lifestyle of the individual. Most people will return at 9 -12 months for a top up.

5. WHAT ARE THE SIDE-EFFECTS OF DERMAL FILLER INJECTION?

You may experience some discomfort from the injections. Discomfort can be minimised by using local anaesthetic or ice before the injection or using dermal fillers with anaesthetic in the same syringe. The side effects of treatment are usually related to the injection procedure and this may include swelling, redness and occasional bruising. Other side effects may include minor lumps, itching or tenderness, which usually pass quickly.
» Hyaluronic Acid Dermal Fillers
1. WHAT ARE HYALURONIC ACID DERMAL FILLER?

Hyaluronic acid (HA) is a naturally occurring substance found in the body that helps to hydrate and add volume to the skin. Naturally occurring HA is broken down by the body in 1-2 days and we lose HA with age.

Hyaluronic acid dermal fillers contain HA which are cross linked so that they stay in the body longer. Since the HA is compatible to the body, HA fillers are safe and do not cause skin allergy when injected into the skin.

2. WHAT ARE THE DIFFERENT HA DERMAL FILLERS?

HA fillers can be either from non animal source or animal source. There are many HA fillers and they differ in terms of longevity, particle size and cross linking.

In Dr Joyce Lim Skin and Laser Clinic, we use only HA fillers that are FDA approved and HSA registered.

3. HOW DOES HA DERMAL FILLER WORK?

HA dermal filler is made up of cross-linked HA which is compatible with the body. It fills up lines and folds and adds volume. Once injected into the skin the HA filler draws in water and the amount remains the same till it is removed by the body. The filler fills up any volume loss or lifts up any lines or depressions.

4. WHO ARE THE IDEAL CANDIDATES FOR HA DERMAL FILLER?

Anyone will benefit from HA dermal fillers. Usually HA dermal filler is injected to treat static lines and folds and volume loss as well as for facial enhancement. Dr Joyce Lim will discuss with you your concerns and decide whether HA dermal filler will work for you

5. WHAT CAN I EXPECT DURING THE HA DERMAL FILLER PROCEDURE?

The area to be treated will be cleaned with antiseptic and iced before the injection. You may feel a tiny pricking sensation as the needle or blunt canula enters the skin. You will notice the anaesthetic result immediately after the procedure. The whole procedure takes 15-20 minutes.

6. WHAT MUST I DO AFTER THE TREATMENT?

You can resume normal activities and makeup immediately after the procedure. Avoid massaging the treated area for at least 24 hours. You can use ice compress to alleviate any swelling or bruising after the procedure

7. WHAT ARE THE SIDE-EFFECTS OF HA DERMAL FILLER PROCEDURE?

Side-effects from HA dermal fillers are related to the treatment procedure. You may have slight redness, swelling or bruising after the treatment. This will clear within a week. The next day there may be swelling at the injection sites.

8. HOW MANY SESSIONS OF HA DERMAL FILLER PROCEDURE DO I NEED?

Generally one treatment will address the aesthetic concerns. You may need several treatments to address different areas. One HA dermal filler effect will last up to one year after the initial injection. Many people will book touch-up treatments or repeat treatments to maintain the way they look. Discuss with Dr Joyce Lim regarding repeat treatments

9. WHAT ARE THE ADVANTAGES OF HA DERMAL FILLER PROCEDURE OVER OTHER FILLER PROCEDURE?

HA dermal filler is safe and results are effective as the HA composition is similar to our natural body HA. There has been no reported allergy or long term side-effects. It is easily removed if you do not like the results
» Radiesse Dermal Filler
1. WHAT ARE RADIESSE DERMAL FILLER?

Radiesse dermal filler is a semi-permanent filler that contains tiny calcium hydroxylapatite (CaHA) microspheres in a water-based gel – both of which are inert and safe to inject into the skin. Over time, the gel is absorbed and the body metabolizes these tiny calcium-based microspheres, leaving behind only your own collagen

2. HOW IS RADIESSE DERMAL FILLER DIFFERENT FROM HA FILLERS?

Radiesse is a stimulatory filler and has FDA approval for aesthetic use since 2006. Radiesse is the only dermal filler to provide both instant results and new collagen formation, for long-lasting results. The gel matrix immediately fills and lifts lines and folds as well as creates volume like HA fillers. In addition, the fine calcium microspheres stimulate new collagen formation for long –lasting volume and lift, giving a 3D lift.

Over time, the body’s natural metabolic processes absorbed the gel and metabolize these tiny calcium microspheres, leaving behind your own natural collagen.

3. WHAT CAN I EXPECT DURING THE RADIESSE DERMAL FILLER PROCEDURE?

The area to be treated will be cleaned with antiseptic and iced before the injection. You may feel a tiny pricking sensation as the needle or blunt canula enters the skin. You will notice the anaesthetic result immediately after the procedure. The whole procedure takes 15-20 minutes.

4. CAN RADIESSE DERMAL FILLER BE FELT UNDER THE SKIN?

Immediately and for a few days, you may feel firmness at the site of the injection. This will eventually be completely integrated and you will not feel it over time.

5. HOW LONG DOES THE RESULT OF RADIESSE DERMAL FILLER LAST?

Generally, the effect lasts over a year. However, individual results are dependent on age, skin type, lifestyle, metabolism as well as the area treated.

6. WHAT ARE THE SIDE-EFFECTS OF RADIESSE DERMAL FILLER PROCEDURE?

As with any dermal filler, the side-effects are related to the injection procedure. There may be pain, bruising, itching, mild swelling, and discoloration – all these will disappear within a few days.

Radiesse dermal filler has been used in more than 3 million aesthetic treatments worldwide since 2012 and Dr Joyce lim has been using Radiesse dermal fillers since 2007.

7. HOW DOES RADIESSE DERMAL FILLER PROCEDURE DIFFER FROM OTHER DERMAL PROCEDURES?

In two head-to-head clinical studies (reported in Dermatology Surgery in 2007 and 2008), Radiesse dermal filler was shown to have significantly greater satisfaction compared to HA dermal fillers. Patients were happier with radiesse dermal filler and were more likely to return for additional treatments.
» Sculptra Dermal Filler
1. WHAT IS SCULPTRA DERMAL FILLER?

Sculptra dermal filler is a semi-permanent filler that contains poly-L –Lactic acid (PLLA), an inert material found in skin suture. This material is biocompatible, biodegradable and resorbable The PLLA acts as a catalyst that stimulates new collagen. Sculptra is useful to fill volume loss in the cheeks, chin, recessed temple area, jowls as well as filling the laugh lines and the marionette lines.

2. HOW IS SCULPTRA DERMAL FILLER DIFFERENT FROM OTHER FILLERS?

Sculptra dermal filler is a stimulatory filler that has been used in Europe since 1999 and has FDA approval for aesthetic use. It is different from other dermal fillers in its unique collagen replenishing process. Initially there is a temporary filling effect due to the volume of the injected filler and this lasts for a week. Then the PLLA stimulates new collagen formation to give you a natural look that can last up to 2 years.

Over time, the body’s natural metabolic processes absorbed the PLLA, leaving behind your own natural collagen.

3. WHAT CAN I EXPECT DURING THE SCULPTRA DERMAL FILLER PROCEDURE?

The area to be treated will be cleaned with antiseptic and iced before the injection. You may feel a tiny pricking sensation as the needle or blunt canula enters the skin. You will notice the anaesthetic result immediately after the procedure. The whole procedure takes 15-20 minutes.

4. WHAT MUST I DO AFTER THE SCULPTRA DERMAL FILLER PROCEDURE?

There is no downtime and you can resume normal activities right after the procedure. You will have to massage the treated area for five minutes, five times a day for the next 5 days. Dr Joyce Lim will give you special after care instructions

5. CAN SCULPTRA DERMAL FILLER BE FELT UNDER THE SKIN?

No. Rarely you may feel small nodules under the skin and these are prevented by adequate massaging after the procedure for a week. These nodules will eventually be completely integrated and you will not feel them over time.

6. WHEN CAN I EXPECT TO SEE THE RESULTS?

You will see results approximately 6 weeks after the procedure. In some patients this may take longer. Generally you will need an average of 3 treatments to see great results. New collagen growth is seen and the skin quality improve resulting in a fresher and younger skin.

7. HOW LONG DOES THE RESULT OF SCULPTRA DERMAL FILLER LAST?

Generally, the effect lasts over two years. However, individual results are dependent on age, skin type, lifestyle, metabolism as well as the area treated.

8. WHAT ARE THE SIDE-EFFECTS OF SCULPTRA DERMAL FILLER PROCEDURE?

As with any dermal filler, the side-effects are related to the injection procedure. There may be pain, bruising, itching, mild swelling, and discoloration – all these will disappear within a few days.

Other side-effects may include small lumps under the skin that are sometimes noticeable when pressing on the treated area. Larger lumps, some with delayed onset with or without inflammation or skin discolouration have been reported. Both the small and large lumps will resolve without treatment.
