» The Fraxel Treatment
1. WHAT IS THE FRAXEL TREATMENT?

The Fraxel treatment is a non-invasive fractional laser treatment that provides a wide range of treatment options to address different skin problems like signs of aging (age spots, fine lines, wrinkles, visible photoaging, sagging skin), skin pigmentation (melasma, brown spots, birthmarks), scars (acne, postsurgical, post traumatic), actinic keratosis and stretch marks. It is also used to improve skin tightening and texture and to rejuvenate the skin of the neck, chest and hands. It is useful to improve loose skin and wrinkles around the eyes.

Fraxel is the pioneer of "fractional photothermolysis", a technology in which thousands of microscopic laser columns - each just one-tenth the diameter of a hair follicle - treat a fraction of the skin at a time without affecting the surrounding tissue. These laser columns stimulate the production of younger, smoother, healthier skin to replace damaged tissue. Fraxel only treats targeted problem areas – so the results are focused and effective.

2. HOW DOES THE FRAXEL WORK?

The Fraxel delivers laser light energy into the skin as the laser hand piece glides over the skin. The light energy creates small microscopic laser columns called microthermal treatment zone (MTZ). The MTZ can be adjusted to different diameters and different depths depending on the Fraxel laser used and the condition being treated. Each MTZ extends from the epidermis to the mid-dermis, allowing the Fraxel laser to treat both superficial and deep problems in the skin

The MTZ stimulates your skin’s natural collagen, rejuvenating skin cells below the surface to help smooth the creases and pockets that cause wrinkles and scars. The MTZ also removes abnormal skin lesions like pigmentation and precancerous skin growths

3. WHAT IS UNIQUE ABOUT THE FRAXEL?

The Fraxel laser is designed to treat only a fraction of the skin at one time and this can be from 5 to 45% of the area, hence the name fractional resurfacing., This means that it spares 55 to 95% of the skin which allows the skin to heal faster and minimised side-effects from the laser. Hence, most patients enjoy minimal adverse effects, little downtime, and complete healing within 1 week.

The Fraxel laser uses a unique delivery technology (the intellegient optical tracking system IOTS) which allows the MTZ to be formed independent of the user speed and accuracy of placement. Multiple MTZs are formed as the IOTs handpiece glides over the skin. This is an advantage over other laser systems that use the stamping mode which may leave uneven delivery of MTZ.

4. WHAT ARE THE DIFFERENT FRAXEL LASERS?

There are two groups of Fraxel treatment – the nonablative Fraxel treatment and the ablative Fraxel treatment

Non-ablative Fraxel treatments improve mild to moderate skin damage with little or no downtime, using the following Fraxel lasers
  • Fraxel 1440nm Clear and Brillant - for prevention and maintenance
  • Fraxel 1550 nm treatment - for mild to severe skin problems, scars
  • Fraxel 1927nm treatment – for mild to severe skin problems, skin pigmentation, precancerous skin lesions

Ablative fraxel laser treatment uses the Fraxel re:pair SST treatment to correct acne scars and moderate – severe photoaging (usually in a single session)

Fraxel re:pair SST treatment is a revolutionary new way to address the most severe signs of skin damage—without the degree of risks, complications and prolonged downtime of other ablative laser procedures. Fraxel re:pair SST treatment uses a high-intensity carbon dioxide (CO2) laser to vaporize unwanted tissue. Due to pioneered fractional technology, the treatment delivers dramatic results with significantly faster recovery time than non-fractional treatments—all in 1-2 treatments.

5. WHAT CAN I EXPECT DURING THE FRAXEL TREATMENT?

The Fraxel laser heats up the skin and so you will feel heat and a prickly sensation. The intensity of heat will depend on the Fraxel laser used and the aggressiveness of the treatment. To mimimise discomfort, a topical anaesthesia will be applied before Fraxel laser treatments

6. WHAT MUST I DO AFTER THE FRAXEL TREATMENT?

You skin may feel a sunburnt like effect for a few hours or a few days depending on the Fraxel laser used. If you have the ablative Fraxel Re:pair treatment , there may be some swelling or pinpoint bleeding for a day or so and your skin will be pinkish for up to a week. Sometimes a fine crusty or bronze appearance may occur as your skin sheds the dead cells as it heals. Rarely scabbing can occur. Dr Joyce Lim will discuss with you on post laser care.

7. WHO ARE THE IDEAL CANDIDATES FOR THE FRAXEL TREATMENT?

Fraxel treatment is for people who want to look good – and want real results – but don’t want the overly change or extended downtime that comes with surgery. Fraxel treatment fits into any lifestyle and your current beauty regimen

Fraxel treatment is effective on
  • Fine lines and wrinkles – like crow’s feet and brow lines
  • Surface scarring – erasing effects of acne and other scarring
  • Pigmentation – minimising the appearance of age spots and melasma
  • Stretch marks – minimise the white streaks
  • Skin tightening – like loose skin around the eyes and on cheeks
  • Sun damage – helping heal dangerous skin damage
  • Actinic Keratosis (AK) – a common pre-cancerous skin condition

8. WHEN CAN I EXPECT TO SEE RESULTS AFTER THE FRAXEL TREATMENT?

Fraxel works with your body’s natural skin cells, so results typically take 1-3 weeks to take effect. Most people will see improvement in skin tone, colour, wrinkles and scar after one session. Best results are seen after 3-5 sessions for the non-ablative Fraxel Restore Dual laser or after 1-2 sessions of the Fraxel re:pair. The effects continue to improve over 12 months.

9. IS THE FRAXEL TREATMENT SAFE?

With a track record of over 1 million treatments, Fraxel technology is proven to be safe and effective for multiple indications. But because skin types and conditions can vary, some side effects are possible.

10. WHAT ARE THE SIDE - EFFECTS AFTER FRAXEL TREATMENT?

Potential side effects include prolonged redness, swelling, blistering, scarring, infection, pigmentary changes (hyperpigmentation), herpes reactivation and acne flare-up.

11. HOW MANY SESSIONS OF FRAXEL TREATMENT DO I NEED?

This depends on your condition and should be discussed at your consultation. Usually 2-5 treatments at 2-4 weeks intervals are required for the Fraxel Dual 1550/1927 Laser.

Less aggressive treatments accomplish the same results, with a few more treatment sessions and less pronounced side-effects.

Dr Lim will use the 1550nm Fraxel laser or the 1927nm Fraxel laser or a combination of both depending on your particular needs.

6. IS THE FRAXEL RE:STORE DUAL LASER TREATMENT PAINFUL?

Most patients describe an increased sensation of heat during the treatment. To minimize discomfort, topical anaesthesia is applied before the procedure and cooling is built directly into the new handpiece for improved patient comfort. After the treatment, most patients feel like they have a sunburn that usually subsides in 1 to 3 hours

7. HOW LONG DOES THE FRAXEL RE:STORE DUAL LASER TREATMENT TAKE?

The treatment takes just 20 to 25 minutes for a full face with additional time for anaesthetic application.

8. WHAT CAN I EXPECT IMMEDIATELY AFTER THE FRAXEL RE:STORE DUAL LASER TREATMENT?

Depending on the level of treatment, you may experience redness or swelling after the procedure which diminishes as quickly as the next day or over the next few days. During the first 24 hours after a treatment, your skin may feel as though it is sunburned, and it will appear pink or red. You can wear make-up to reduce the sunburned appearance.

Your skin will naturally bronze over the next few days or week. As with sunburn, your skin will flake and exfoliate normally, revealing fresher, glowing and smoother looking and feeling skin. Use a moisturizer to reduce the appearance of dry flakes.

Dr Joyce Lim will work with you to identify the appropriate level of treatment and the preferred downtime

9. WHEN DO RESULTS APPEAR AFTER THE FRAXEL RE:STORE DUAL LASER TREATMENT?

You can expect two levels of results, immediate and progressive. Immediately after initial healing is complete, the surface of your skin will feel softer and smoother, look brighter and show a more even skin tone. Progressive results occur over the next three to six months as the deeper layers of the skin continue to create new collagen.

10. ARE THE RESULTS LONG LASTING?

Results from the Fraxel re:store Dual treatment are long lasting depending on your lifestyle, age, skin condition and skin care regime. Once your skin is restored, Dr Joyce Lim will give you specific advice on your skin care regime based on your individual needs.

11. HOW MANY FRAXEL RE:STORE DUAL TREATMENTS WILL I NEED?

Multiple treatments are needed for the desired outcomes. The specific number of treatments is tailored to your skin problems, your treatments goals and your life-style.

Clinical studies suggest that on average, an effective treatment regimen is 3 to 5 sessions spaced about 2 to 4 weeks apart. Depending on your condition and schedule, you and Dr Joyce Lim may choose to space treatment sessions further apart. Results are immediate and progressive, with optimal improvement usually visible in 2 to 3 months. This time frame and treatment structure allow for complete healing and the production of new collagen and elastin to replace damaged tissue.

12. IS THE FRAXEL RE:STORE DUAL LASER TREATMENT SAFE?

Fraxel treatment is a safe option backed by extensive research. As with any laser procedures there is always some risk and side-effects. Dr Joyce Lim will discuss this with you prior to the treatments.

Swelling and redness are the most common side effects from a Fraxel Re:store DUAL 1550/1927 treatment. Any swelling is typically minimal and subsides within a day or two, and any redness typically fades within a few days. Other temporary side effects may include minor itching, dry skin, peeling or flaking, and a bronzed skin appearance. There is a limited risk of infection, hyperpigmentation, or scarring. The intensity and duration of side effects depend on the aggressiveness of a treatment and on your particular healing characteristics. Dr Joyce Lim will provide specific instructions about what to expect and how to minimize any side effects.

13. HOW DOES IT COMPARE WITH OTHER LASER TREATMENTS?

Compared to other lasers, the Fraxel Re:store Dual laser can treat skin with more precision and depth. Treatment can be easily individualised to your skin problems and skin type. By design, Fraxel treatments can target between 5-50% of the skin’s surface, leaving the remaining 50-95% available to quickly heal the treated areas. Since Fraxel Dual treatment is so precise, it is safe and effective on delicate skin areas like the neck, chest and hands, in addition to the face.

The Fraxel Re:store Dual treatment is a safe, non-ablative laser skin procedure that can resurface damaged skin and uncover youthful skin. Specific results can be found in the case gallery on the Fraxel website at www.fraxel.com
» Fraxel Re:pair
1. WHAT IS THE FRAXEL RE:PAIR ?

The Fraxel Re:pair SST is an ablative laser and the treatment uses a carbon dioxide (CO2) laser to vaporize microscopic columns of tissue deep within the skin. Fraxel Re:pair SST treatment is a more aggressive procedure and is used to treat the most severe signs of skin damage and acne scars. Results are achieved in 1-2 treatments and wound care recovery time (usually 24-72 hours) is generally longer than other Fraxel treatments.

2. HOW DOES THE FRAXEL RE:PAIR LASER WORK?

The Fraxel Re:pair laser uses a patented fractional technology to target damaged skin with microscopic laser columns that penetrate deep into the skin. The laser treats only a fraction of skin at a time, leaving the surrounding tissues untouched, which promotes rapid healing. This stimulates your body’s own natural healing process, replacing the old and damaged cells with fresh, glowing healthy skin.

The Fraxel Re:pair provides a non-invasive treatment and resurfaces your skin by stimulating the growth of new, healthy skin cells from the inside out. It allows for deep and superficial resurfacing of larger and multiple body areas in a single session.

3. WHAT ARE THE BENEFITS OF FRAXEL RE:PAIR TREATMENT?

After a Fraxel Re:pair treatment, you can expect to have improved skin tone and texture and a smoother, fresher looking skin. The benefits are
  • Significant results in a single treatment
  • Smoother, more even skin tone and texture
  • Softening of deep frown lines and wrinkles
  • Improved appearance of redness caused by sun damage
  • Improvement in acne scars and keloids

4. WHAT FDA CLEARANCES HAS THE FRAXEL RE:PAIR TREATMENT RECEIVED?

Fraxel re:pair SST laser has received FDA clearance for many indications on and off the face, including:
  • Rhytides(wrinkles) improvement
  • Pigmented lesions clearance
  • Improvement in moderate to severe wrinkles and furrows in lax skin
  • Fine lines improvement
  • Textural irregularities improvement
  • Vascular dyschromia improvement

5. HOW IS FRAXEL RE:PAIR TREATMENT PERFORMED AND DOES THE TREATMENT HURT?

First, your skin will be cleansed. Then, 30- 60 minutes prior to treatment, a topical numbing agent will be applied to the treatment area. Most patients describe an increased sensation of heat during the treatment, and there may be some discomfort. However, unlike the experience in traditional CO2 treatments, you will feel quite comfortable immediately after a Fraxel re:pair SST treatment. The procedure takes approximately 20-30 minutes for a full face.

6. HOW LONG DO THE POSITIVE EFFECTS OF FRAXEL RE:PAIR SST TREATMENT LAST?

The results of Fraxel re:pair SST treatment are both immediate and progressive. Soon after the treatment, the surface of your skin will feel softer, look brighter and show more even tone. The next 3 to 6 months will bring more improvement as the deeper layers of the skin continue to heal.

7. WHAT SIDE EFFECTS SHOULD I EXPECT AFTER A FRAXEL RE:PAIR TREATMENT AND WHAT ARE THE RISKS?

Depending on the aggressiveness of your treatment, there might be some pinpoint bleeding following the procedure, as well as some oozing for up to 48 hours until the wounds close. The most common side effects are edema (swelling), which usually subsides after a week, and erythema (redness), which may last up to a month, depending on the aggressiveness of treatment. Other temporary side effects may include minor itching, dry skin, peeling or flaking, oozing and crusting. As with all ablative laser procedures, there is an elevated risk of delayed wound healing, infection, pigmentary changes (hyperpigmentation) and/or scarring.

» Fraxel Peel
WHAT IS THE FRAXEL PEEL?

The fraxel peel is a skin resurfacing treatment to improve skin texture, increase skin brightness and smoothness and to rejuvenate the skin. The depth of the peel will depend on the skin problems. Superficial skin problems like pigmentation and fine wrinkles will improve with the Fraxel Dual laser. Wrinkles, sagging skin and acne scars can be erased and improved with the Fraxel Restore or the Fraxel Re:pair laser. To get optimal results you will need several sessions. Dr Joyce Lim will tailor the right treatment for you and your skin problems.
» Fraxel Scar Treatment
WHAT CAN THE FRAXEL LASER DO FOR SCARS?

There are many types of scars – the surgical scar, the traumatic scars, keloids and hypertrophic scars and acne scars. The fraxel laser can improve these scars and the choice of the fraxel laser will depend on the type of scar and the depth of the scar. Both the Fraxel Restore and the Fraxel Re:pair lasers work well on scars. After repeated treatment sessions, the scars are flatten and the skin is smoother. The brown or dark colour is also improved after fraxel laser treatments. However the red colour will require the addition of Vbeam vascular laser treatments

There are many laser treatments for acne scars and the most effective laser treatment is still the fractional lasers – either the fractional ablative CO2 laser (Fraxel Re:pair) or the fractional non ablative laser (Fraxel Restore 1550nm laser)

FRAXEL TREATMENTS
  • Over the years, the skin is damaged by both environmental (sun, pollution) and behavioral (stress, fatigue, smoking) factors, keeping people from looking their best and how they may feel inside. Today, Fraxel treatment offers the most advanced new microlaser restoration treatment – one that actually awakens the subsurface cells so they revitalize themselves naturally, returning skin to its original beauty.

  • The Fraxel laser has been approved by the FDA to treat multiple skin conditions, including age/sun spots, acne scarring, melasma (the mask of pregnancy) and eye wrinkles (crow’s feet). It’s safe, easy and delivers remarkable results.

  • Fraxel treatment repairs damaged skin by treating only a small fraction of skin at a time, leaving the area surrounded by unaffected, healthy tissue. This "fractional" treatment allows the skin to heal much faster than if the entire area were treated at once, using the body's natural healing process to create new, healthy, tighter tissue to replace skin imperfections.

  • Fraxel treatment is different. It is the only fractional laser system that safely penetrates the skin down to the dermis. The Fraxel laser has the ability to penetrate fractions of skin, spot by spot, without breaking the skin’s protective outer barrier. The result is natural stimulation of new collagen without the fear of downtime and risks associated with traditional ablative treatments. The product provides a highly effective, predictable and safe non-ablative treatment for patients with acne scarring, melasma or hormonal skin discoloration, periorbital wrinkles and sun and age spots.

    • The Fraxel laser passes through the top layer of skin like light through glass, reaching into the dermis below to create thousands of tiny, microscopic areas of thermal damage that stimulate the body’s own wound healing response, notably new collagen production and replacement of damaged skin surfaces with new epidermal skin.

  • Fraxel treatment is performed using topical anesthesia in the doctor’s office. A typical facial treatment is completed in about 30 minutes. While results vary from patient to patient, results are typically achieved in an average of 3 to 5 treatment sessions spaced about 2 to 4 weeks apart, and many patients notice a marked improvement sooner.

AGE/SUN SPOTS
  • While many people are taking precautions such as sunscreen and protective clothing, the damage caused by decades of harmful sun exposure remains. Today, Fraxel treatment offers the most advanced new microlaser restoration treatment – one that actually awakens the subsurface cells so they revitalize themselves naturally, reducing the appearance of sun and age spots and returning skin to its original beauty.
  • Age spots are flat, uneven brown patches that appear on the parts of the body that are most regularly exposed to the sun – most commonly the face, chest, hands, shoulders and arms. They are also known as sun spots or liver spots.
  • When you spend time in the sun, your skin reacts by producing pigment called melanin for protection. The purpose of the melanin is to shield your skin from the sun’s ultra violet rays. Sometimes the increased pigmentation results in darkened spots – an indication of aged skin that can make a person look much older than they actually are.

ACNE SCARRING
  • Acne scarring can have a profound impact on people’s self-esteem and life-outlook, particularly as adults enter work and social settings with these often painful memories of their teenage years. Today, Fraxel offers the most advanced new microlaser restoration treatment – one that actually awakens the subsurface cells so they revitalize themselves naturally, reducing the appearance of acne scars and returning skin to its original beauty.
  • When acne advances to the point of injuring our tissue, it can result in a scar – a discolored fibrous tissue with abnormal collagen function that may appear as a red raised area or indented mark. To successfully treat this type of scar, the top layer of skin (epidermis) must be removed, and the production of new collagen must be stimulated to improve the skin’s overall tone and texture.
  • Traditionally, topical agents, abrasion therapies, surgery and conventional lasers have been used to treat acne scarring. Unfortunately, most of these treatments achieve only moderate results or are very aggressive, resulting in a long, painful recovery period.
  • A monumental breakthrough came only recently when the FDA approved Fraxel treatment for acne scars after studies showed that the new laser could be far more effective than traditional therapies. The powerful, yet precise laser effectively encourages the skin to create new collagen to replace the scarred epidermis.

MELASMA
  • While nothing compares to the joy of motherhood, pregnancy can take a toll on women’s skin. Between stretch marks, melasma and hormonal changes, there are some lasting reminders that women would prefer to erase. Fraxel treatment can help to reverse the unsightly changes associated with Melasma and return skin to its original beauty.
  • Melasma is a very common disorder that is estimated to affect about 6 million women in the United States. Melasma typically occurs as brown spots and darkened pigmentation on sun-exposed facial areas, notably the cheeks, forehead, upper lip and nose, and is most common in women of child-bearing age and among male and female adults of dark-skinned races.
  • Fraxel treatment has been shown to be very effective in treating persistent melasma that has otherwise been unresponsive to traditional therapies, such as bleaching creams, microdermabrasion, selective photothermolysis laser and light therapies, and laser ablation.
  • To understand the Fraxel laser’s unique mechanism of action, think of your skin as a wall made up of tiny tiles. If a patient has melasma, every third tile on this wall would be brown in color, representing the brown splotches that are characteristic of this condition. With fractional resurfacing, many of these brown tiles are “zapped” and eliminated making the appearance more uniform.

WRINKLES AROUND THE EYES
  • While wrinkles around the eyes may signify a life rich with laughter and emotional experiences, many people would prefer to keep the memories but lose the crow’s feet! Fraxel laser works to rebuild and restore the skin naturally in the area around the eye, reducing the appearance of fine lines and wrinkles and returning skin to its original beauty.
  • The skin around your eyes is much more delicate than other areas of the body and tends to age more rapidly. The tiny wrinkles that develop, often called crow’s feet, develop largely from years of contracting the muscles that display our emotions when we smile, squint, deeply consider, show surprise or laugh.
  • There are countless cosmetic strategies for reducing crow's feet, including acid peels, injectable fillers, collagen, dermabrasion or traditional laser treatment. However, in many cases, the results are unsatisfactory, painful or too expensive, as the process must be repeated a number of times each year in order to be effective.
  • The real breakthrough came recently when the FDA approved Fraxel treatment for wrinkles around the eyes after studies showed that the new laser was extremely effective in improving the appearance of fine lines and wrinkles.
